Description
Overview of the medicine
Oracyn-K 125 mg Suspension contains phenoxymethylpenicillin potassium, a penicillin antibiotic used to treat various bacterial infections, primarily in children. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Typically 250-500 mg every 6-8 hours for adults, often using higher strength formulations. For children, 12.5-25 mg/kg/day in 3-4 divided doses (e.g., 125 mg (5 mL) every 6-8 hours).
Elderly
Dosage should be adjusted based on renal function.
Renal_impairment
Dose adjustment may be necessary in severe renal impairment (CrCl < 10 mL/min).
How to Take
Administer orally. Shake well before use. Can be taken with or without food, but absorption is better on an empty stomach (1 hour before or 2 hours after meals).
Mechanism of Action
Phenoxymethylpenicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within 30 minutes to 1 hour.
Excretion
Mainly excreted by the kidneys via tubular secretion and glomerular filtration.
Half life
Approximately 30-60 minutes.
Absorption
Well absorbed from the gastrointestinal tract, especially when taken on an empty stomach. Peak plasma concentrations are achieved within 30-60 minutes.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to penicillin or any component of the formulation
- •History of severe allergic reaction to other beta-lactam antibiotics
Drug Interactions
Methotrexate
Penicillin may reduce methotrexate excretion, increasing its toxicity.
Tetracyclines
May decrease the bactericidal effect of penicillin.
Oral anticoagulants (e.g., Warfarin)
May increase anticoagulant effect by altering gut flora and reducing Vitamin K synthesis.
Storage
Store dry powder below 30°C, protected from light and moisture. Once reconstituted, store in a refrigerator (2-8°C) and do not freeze.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and in rare cases, neuromuscular hypersensitivity, or seizures. Treatment is primarily supportive and symptomatic. Gastric lavage and hemodialysis may be considered in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe during pregnancy. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for adverse effects like diarrhea or rash.
