Description
Overview of the medicine
Oracyn-K 250 mg Tablet contains Phenoxymethylpenicillin Potassium, an antibiotic belonging to the penicillin group. It is used to treat a wide range of bacterial infections by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Usually 250-500 mg every 6-8 hours, depending on the severity of the infection. For streptococcal pharyngitis, 250 mg every 6-8 hours for 10 days.
Elderly
No specific dosage adjustment is typically required unless severe renal impairment is present. Doses similar to adults.
Renal_impairment
Dosage adjustment may be necessary for severe renal impairment (creatinine clearance <10 mL/min). Consult a physician for specific guidance.
How to Take
Oracyn-K tablets should be taken orally, preferably on an empty stomach (at least 1 hour before or 2 hours after meals) to ensure maximum absorption. Take with a full glass of water. Complete the entire prescribed course of treatment.
Mechanism of Action
Phenoxymethylpenicillin Potassium works by inhibiting the synthesis of the bacterial cell wall, leading to bacterial lysis and death. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, which prevents the cross-linking of peptidoglycan units, a critical step in cell wall formation.
Pharmacokinetics
Onset
Rapid, typically within 30-60 minutes.
Excretion
Primarily via renal excretion (60-80% unchanged drug) within 4-5 hours.
Half life
Approximately 0.5 to 1 hour.
Absorption
Well absorbed from the gastrointestinal tract; however, food may decrease absorption. Peak plasma concentrations are achieved within 30-60 minutes.
Metabolism
Minimal hepatic metabolism; about 10-30% is metabolized to inactive compounds.
Side Effects
Contraindications
- •Known hypersensitivity to penicillin or any component of the formulation.
- •History of severe allergic reactions to other beta-lactam antibiotics (e.g., cephalosporins).
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring close monitoring of INR.
Methotrexate
Penicillin may reduce methotrexate excretion, leading to increased toxicity.
Tetracyclines
May antagonize the bactericidal effect of penicillin.
Oral Contraceptives
Rarely, penicillins may reduce the efficacy of oral contraceptives.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and in rare cases, neuromuscular excitability or seizures. Treatment is largely supportive. Hemodialysis may be considered in severe cases of renal impairment.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use in pregnancy should only be considered if clearly needed. Phenoxymethylpenicillin is excreted in small amounts into breast milk; caution should be exercised when administered to a nursing mother.
