Description
Overview of the medicine
Oracyn-K 500 mg Tablet contains Phenoxymethylpenicillin Potassium, a penicillin antibiotic used to treat various bacterial infections such as throat infections, ear infections, skin and soft tissue infections, and for prophylaxis of rheumatic fever.
Uses & Indications
Dosage
Adults
250-500 mg every 6-8 hours, depending on the severity of infection.
Elderly
Similar to adults, but dosage adjustment may be necessary in cases of renal impairment.
Renal_impairment
Dose adjustment required based on creatinine clearance. For severe impairment, dose interval may be extended.
How to Take
Oracyn-K tablets should be taken orally with water. For optimal absorption, it is best taken on an empty stomach (at least 30 minutes before or 2 hours after meals), but it can be taken with food to reduce gastrointestinal upset.
Mechanism of Action
Phenoxymethylpenicillin works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Rapid, usually within an hour of administration.
Excretion
Primarily excreted unchanged by the kidneys (renal excretion), with a small amount excreted in bile.
Half life
Approximately 30-60 minutes.
Absorption
Well absorbed orally, though absorption may be decreased by food. Peak plasma concentrations typically occur within 30 to 60 minutes after oral administration.
Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to penicillin or any component of the formulation.
- •History of severe allergic reaction to other beta-lactam antibiotics (e.g., cephalosporins, carbapenems).
Drug Interactions
Probenecid
Increases and prolongs penicillin plasma levels.
Methotrexate
May increase methotrexate toxicity by reducing its renal excretion.
Tetracyclines
May antagonize the bactericidal effect of penicillin.
Oral Contraceptives
Penicillins may reduce the effectiveness of oral contraceptives, although this interaction is controversial.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe nausea, vomiting, diarrhea, abdominal pain, and neuromuscular hypersensitivity (e.g., agitation, hallucinations, seizures). Management involves symptomatic and supportive care. Gastric lavage may be considered if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk, but there are no adequate and well-controlled studies in pregnant women. Generally considered safe for use during pregnancy and lactation, as it is excreted in breast milk in small amounts, but usually not enough to harm the infant.
