Overview of the medicine
Oticef 125 mg Suspension contains Cefixime, a third-generation cephalosporin antibiotic used to treat a variety of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg daily as a single dose or 200 mg every 12 hours. For suspensions, typically for specific infections or for patients unable to take tablets.
Elderly
No specific dosage adjustment is generally required for elderly patients, unless there is significant renal impairment.
Renal_impairment
Dosage adjustment is necessary for patients with severe renal impairment (creatinine clearance < 30 mL/min). A dose of 200 mg daily is recommended.
How to Take
Shake the suspension well before each use. It can be taken with or without food. Complete the full course of medication as prescribed by your doctor.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis, leading to cell lysis and death.

Onset
Peak plasma concentrations typically within 2-6 hours after oral administration.
Excretion
Excreted primarily via the kidneys (about 50% as unchanged drug) and also through bile.
Half life
Plasma half-life is usually 3-4 hours, but can extend to 9 hours in some cases.
Absorption
Oral bioavailability is about 40-50%, not significantly altered by food. Peak plasma concentrations are achieved in 2 to 6 hours.
Metabolism
Not extensively metabolized in the body. Minor metabolism may occur.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase cefixime peak plasma levels and AUC.
Carbamazepine
Increased carbamazepine levels, requiring dose adjustment.
Diuretics (e.g., Furosemide)
Concomitant use with nephrotoxic drugs may increase the risk of renal toxicity.
Oral Anticoagulants (e.g., Warfarin)
Increased prothrombin time (INR) and bleeding risk. Monitor coagulation parameters closely.
Storage
Store the dry powder at room temperature, below 30°C, protected from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard any unused portion after the specified period (e.g., 7 or 14 days). Do not freeze.
Overdose
In case of overdose, supportive and symptomatic treatment is recommended. Hemodialysis or peritoneal dialysis may not be effective in removing cefixime from the body.

Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use in pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; use with caution in breastfeeding mothers.
