Overview of the medicine
Oticef 500 mg Capsule contains Cefadroxil, a first-generation cephalosporin antibiotic used to treat various bacterial infections, including urinary tract infections, skin and soft tissue infections, and pharyngitis/tonsillitis.
Uses & Indications
Dosage
Adults
For most infections, 500 mg to 1 g once or twice daily, depending on the severity and type of infection. For uncomplicated UTIs, 1-2 g/day in single or divided doses. For skin/soft tissue infections, 1 g/day in single or divided doses. For pharyngitis/tonsillitis, 1 g/day in single or divided doses for 10 days.
Elderly
Similar to adult dosage, but caution and dosage adjustment may be needed based on renal function.
Renal_impairment
Dosage must be adjusted based on creatinine clearance. For CrCl 50-25 mL/min, 500 mg every 12 hours; for CrCl 25-10 mL/min, 500 mg every 24 hours; for CrCl <10 mL/min, 500 mg every 36 hours. A loading dose of 1000 mg may be given for severe infections.
How to Take
Take orally with a glass of water, with or without food. Food may help reduce potential gastrointestinal upset.
Mechanism of Action
Cefadroxil is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.

Well absorbed orally (over 90%), even in the presence of food, reaching peak plasma concentrations within 1-2 hours.
Metabolism
Not extensively metabolized in the liver; it remains largely unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefadroxil, other cephalosporins, or any component of the formulation.
- History of severe hypersensitivity reactions to penicillins or other beta-lactam antibiotics (due to potential cross-reactivity).
Drug Interactions
Probenecid
May decrease the renal excretion of cefadroxil, leading to increased and prolonged plasma concentrations.
Aminoglycosides/Loop Diuretics
Concomitant use with other nephrotoxic drugs may increase the risk of nephrotoxicity.
Oral Anticoagulants (e.g., Warfarin)
Cephalosporins may enhance the anticoagulant effect of warfarin, increasing the risk of bleeding. INR monitoring is recommended.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, diarrhea, and convulsions. Management is primarily supportive. Hemodialysis may be useful in the event of severe renal impairment or massive overdose.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Cefadroxil is excreted in small amounts into breast milk; caution should be exercised when administered to a nursing mother, though generally considered compatible with breastfeeding.
