Description
Overview of the medicine
Oxofylline is a methylxanthine derivative used as a bronchodilator to treat respiratory conditions like asthma and COPD. It helps relax the smooth muscles in the airways, making breathing easier.
Uses & Indications
Dosage
Adults
Usually 400 mg orally twice daily, or as directed by a physician. Doses should be individualized.
Elderly
Lower doses or less frequent administration may be necessary due to reduced metabolism and excretion. Start with 200 mg twice daily and adjust as needed.
Renal_impairment
Dose adjustment may be required; caution is advised. Consult a physician.
How to Take
Take the tablet whole with water, preferably after food to minimize gastrointestinal upset. Do not crush, chew, or break the tablet.
Mechanism of Action
Oxofylline acts primarily by inhibiting phosphodiesterase enzymes, leading to an increase in intracellular cyclic AMP (cAMP). Increased cAMP causes relaxation of bronchial smooth muscles, leading to bronchodilation. It also has some anti-inflammatory effects and may modulate respiratory muscle function.
Pharmacokinetics
Onset
Bronchodilatory effects generally begin within 30-60 minutes after oral administration.
Excretion
Excreted mainly by the kidneys, with a small portion excreted unchanged in urine. Metabolites are also renally excreted.
Half life
Approximately 7-9 hours in adults, but can vary with age, smoking status, and liver function.
Absorption
Well absorbed orally, with peak plasma concentrations typically reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes (e.g., CYP1A2, CYP2E1, CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to oxofylline or other xanthine derivatives (e.g., theophylline, caffeine)
- •Acute myocardial infarction
- •Severe uncontrolled arrhythmias
- •Active peptic ulcer disease
Drug Interactions
Beta-blockers
May reduce the bronchodilatory effect of oxofylline.
Rifampicin, Phenobarbital, Phenytoin
May decrease oxofylline levels, potentially reducing efficacy.
Cimetidine, Erythromycin, Ciprofloxacin, Oral contraceptives
May increase oxofylline levels, leading to increased risk of side effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, abdominal pain, restlessness, insomnia, headache, tremors, convulsions, tachycardia, and cardiac arrhythmias. In severe cases, seizures and cardiac arrest may occur. Management involves supportive care, gastric lavage, activated charcoal, and symptomatic treatment for seizures and arrhythmias.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the benefits outweigh the potential risks. Oxofylline is excreted into breast milk; caution should be exercised when administered to a nursing mother. Consult your doctor.
