Description
Overview of the medicine
Oxycort 100 mcg Inhalation Capsule contains Budesonide, a potent synthetic corticosteroid for inhalation. It is primarily used to reduce inflammation in the airways for the treatment of asthma and chronic obstructive pulmonary disease (COPD). It is not for acute relief of bronchospasm.
Uses & Indications
Dosage
Adults
Asthma: 200-400 mcg twice daily. COPD: 200 mcg twice daily. Dosage may be adjusted based on individual response and severity.
Elderly
No specific dosage adjustment is generally required, but caution is advised due to potential for comorbidities.
Renal_impairment
No specific dosage adjustment recommended, as Budesonide is primarily metabolized in the liver.
How to Take
For oral inhalation only using a dry powder inhaler device (e.g., Turbuhaler, Rotahaler). Do not swallow the capsule. After inhalation, rinse mouth with water to minimize risk of oral candidiasis.
Mechanism of Action
Budesonide is a glucocorticosteroid that exerts its anti-inflammatory effects by binding to glucocorticoid receptors in the cytoplasm of target cells. This complex then translocates to the nucleus, modifying gene expression to inhibit the synthesis of inflammatory mediators (e.g., cytokines, chemokines) and upregulate anti-inflammatory proteins. This leads to a reduction in airway hyperresponsiveness, edema, and mucus production.
Pharmacokinetics
Onset
Within hours for initial effects; full therapeutic effect typically achieved after 1-2 weeks of regular use.
Excretion
Excreted in urine and feces as metabolites.
Half life
Approximately 2-3 hours.
Absorption
Approximately 15% of an inhaled dose of Budesonide is systemically absorbed. Peak plasma concentration is reached within 30 minutes to 2 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- •Acute bronchospasm or status asthmaticus.
- •Hypersensitivity to Budesonide or any components of the capsule.
Drug Interactions
Ketoconazole, Itraconazole, Ritonavir
May increase systemic exposure to Budesonide due to CYP3A4 inhibition, potentially leading to corticosteroid side effects. Use with caution.
Other CYP3A4 inhibitors (e.g., grapefruit juice)
May also increase systemic exposure to Budesonide.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely to cause clinically significant problems. Chronic overdose may lead to signs and symptoms of hypercorticism. Treatment should be supportive and symptomatic. Dosing should be reduced gradually under medical supervision.
Pregnancy & Lactation
Pregnancy Category B. Use only if the potential benefit justifies the potential risk to the fetus. Budesonide is excreted in breast milk; however, at therapeutic doses, effects on the breastfed infant are unlikely. Consult a doctor.
