Description
Overview of the medicine
Budesonide is an inhaled corticosteroid used to manage asthma and COPD. It reduces inflammation in the airways, making breathing easier.
Uses & Indications
Dosage
Adults
Asthma: 200-400 mcg twice daily. COPD: 200 mcg twice daily. Dosage adjusted based on individual response.
Elderly
No specific dose adjustment required, but monitor for systemic corticosteroid effects, especially with higher doses.
Renal_impairment
No specific dose adjustment required. Budesonide is primarily metabolized by the liver.
How to Take
Administered via a dry powder inhaler device. The capsule should be placed in the device, and the patient should inhale deeply and steadily through the mouthpiece. Do not swallow the capsule. Rinse mouth with water after each use.
Mechanism of Action
Budesonide acts locally in the lungs as a potent glucocorticoid. It binds to glucocorticoid receptors, inhibiting the release of inflammatory mediators and suppressing immune cell function, thereby reducing airway inflammation and hyperresponsiveness.
Pharmacokinetics
Onset
Onset of action within hours; full therapeutic effect usually seen within 1-2 weeks of regular use.
Excretion
Primarily excreted in urine and feces as metabolites.
Half life
Approximately 2-3 hours (systemic plasma half-life).
Absorption
Rapidly absorbed from the lungs. Systemic bioavailability is low due to extensive first-pass metabolism in the liver.
Metabolism
Extensively metabolized in the liver by CYP3A4 to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Budesonide or any component of the formulation.
- •Acute bronchospasm (Budesonide is not indicated for rapid relief of bronchospasm).
Drug Interactions
Other Inhaled Corticosteroids
Increased risk of systemic corticosteroid effects if used concurrently with other systemic or inhaled corticosteroids.
Strong CYP3A4 Inhibitors (e.g., Ketoconazole, Itraconazole, Ritonavir)
May significantly increase plasma levels of Budesonide, potentially leading to systemic corticosteroid effects. Concomitant administration should be avoided or dose adjusted with careful monitoring.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely to cause clinically significant problems. Chronic overdose may lead to systemic corticosteroid effects (e.g., Cushing's syndrome, adrenal suppression). Management involves gradual reduction of dosage and supportive care.
Pregnancy & Lactation
Pregnancy Category B. Use only if the potential benefit outweighs the potential risk. Budesonide is generally considered safe during lactation as minimal transfer into breast milk occurs.
