Description
Overview of the medicine
Oxycort 400 mcg Inhalation Capsule is a combination medicine containing Budesonide, an inhaled corticosteroid, and Formoterol, a long-acting beta2-agonist. It is used to prevent and control symptoms of asthma and chronic obstructive pulmonary disease (COPD) by reducing inflammation and relaxing airways.
Uses & Indications
Dosage
Adults
Asthma: Usually 1 inhalation (Budesonide 400 mcg / Formoterol 12 mcg) twice daily. COPD: 1 inhalation (Budesonide 400 mcg / Formoterol 12 mcg) twice daily.
Elderly
No specific dosage adjustment required, but monitor for adverse effects.
Renal_impairment
No specific dosage adjustment recommended.
How to Take
For inhalation use only with a suitable inhaler device (e.g., Turbuhaler, Novolizer). Do not swallow the capsule. Rinse mouth with water after each dose to prevent oral thrush.
Mechanism of Action
Budesonide acts as an anti-inflammatory by reducing the synthesis and release of inflammatory mediators and inhibiting inflammatory cell function. Formoterol selectively stimulates beta2-adrenergic receptors in the bronchial smooth muscle, leading to bronchodilation. The combination provides both anti-inflammatory and bronchodilator effects.
Pharmacokinetics
Onset
Formoterol: 1-3 minutes. Budesonide: Therapeutic effect typically seen within a few days to a week.
Excretion
Mainly excreted in urine and feces as metabolites.
Half life
Budesonide: Approximately 2-3 hours. Formoterol: Approximately 8-10 hours.
Absorption
Budesonide: Rapidly absorbed from the lung, 10-25% systemic bioavailability. Formoterol: Rapidly absorbed from the lung, peak plasma concentration within 10-30 minutes.
Metabolism
Both Budesonide and Formoterol are extensively metabolized in the liver, primarily via CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Budesonide, Formoterol, or any excipients
- Acute asthma attacks or acute exacerbations of COPD
Drug Interactions
Beta-adrenergic blockers (e.g., propranolol)
Can antagonize the effect of formoterol.
Strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
Can increase systemic exposure to budesonide.
QTc-prolonging drugs (e.g., some antidepressants, antihistamines)
Increased risk of ventricular arrhythmias with formoterol.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms may include headache, tremor, palpitations, tachycardia, and adrenal suppression. Treatment is supportive and symptomatic. Consult a doctor immediately.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Budesonide is generally considered safe. Formoterol should be used with caution. Breastfeeding: Small amounts of budesonide and formoterol may pass into breast milk; weigh benefits against risks.
