Overview of the medicine
P-Gut 20 mg tablet contains Pantoprazole, a proton pump inhibitor used to reduce the amount of acid produced in the stomach. It is prescribed for various acid-related conditions like heartburn, acid reflux, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Typically 20-40 mg once daily, depending on the condition. For GERD, 20-40 mg daily for 4-8 weeks. For Zollinger-Ellison Syndrome, initial dose 80 mg once daily, adjusted as needed.
Elderly
No specific dosage adjustment needed, but monitor for renal/hepatic impairment.
Renal_impairment
No dosage adjustment needed.
How to Take
Take whole, do not chew or crush. Usually taken once daily in the morning before food.
Mechanism of Action
Pantoprazole inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby blocking the final step of acid secretion. This leads to a reduction in both basal and stimulated acid secretion.

Onset
Within 2.5 hours; full effect within 2-4 days.
Excretion
Mainly renal (approx. 80%) and biliary/fecal (approx. 20%).
Half life
Approximately 1 hour.
Absorption
Well absorbed after oral administration, bioavailability is about 77%.
Metabolism
Extensively metabolized in the liver via CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to pantoprazole or other substituted benzimidazoles
- •Concurrent use with atazanavir or nelfinavir
Drug Interactions
Warfarin
Increased INR and prothrombin time.
Methotrexate
Increased plasma concentration of methotrexate.
Atazanavir/Nelfinavir
Reduced bioavailability, not recommended.
Ketoconazole/Itraconazole
Reduced absorption due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, tachycardia, nausea, headache. Treatment is symptomatic and supportive. Pantoprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk, use with caution.
