Overview of the medicine
P-gut 40 mg Injection is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions like heartburn, acid reflux, and peptic ulcers, especially in patients who cannot take oral medication.
Uses & Indications
Dosage
Adults
Typically 40 mg once daily by intravenous infusion over 15 minutes, or as directed by a physician. The duration of therapy depends on the condition being treated.
Elderly
No dose adjustment generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment required for renal impairment.
How to Take
For intravenous administration only. Administer slowly over a period of 15 minutes. It should be reconstituted with 10 mL of 0.9% sodium chloride injection and can be further diluted with 0.9% sodium chloride injection, 5% dextrose in water, or lactated Ringer's solution.
Mechanism of Action
Pantoprazole is a substituted benzimidazole that selectively inhibits the H+/K+-ATPase enzyme (proton pump) in the parietal cells of the stomach. This inhibition leads to a dose-dependent and long-lasting suppression of gastric acid secretion.

Excretion
Mainly excreted in urine (approx. 80%) and a smaller proportion in feces (approx. 18%) as metabolites.
Half life
Approximately 1 hour (terminal half-life), but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after intravenous administration, reaching peak plasma concentrations quickly.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 (CYP) system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Known hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with rilpivirine-containing products.
Drug Interactions
Warfarin
Increased INR and prothrombin time, monitor carefully.
Clopidogrel
Reduced antiplatelet effect of clopidogrel (clinical significance debated, but concurrent use generally discouraged).
Methotrexate
Increased methotrexate levels, especially high-dose methotrexate.
Atazanavir, Nelfinavir, Rilpivirine
Reduced absorption of these antiviral drugs, concurrent use generally contraindicated or requires careful monitoring/alternative therapy.
Storage
Store below 25°C (77°F). Protect from light. Keep out of reach of children. Reconstituted solution is stable for up to 6 hours at room temperature.
Overdose
Limited experience with overdose. Symptoms may include lethargy, ataxia, tremor, convulsions. Treatment is symptomatic and supportive. Pantoprazole is not significantly removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Excreted in breast milk, so caution should be exercised when administered to a nursing mother.
