Overview of the medicine
P-gut 40 mg Tablet contains Pantoprazole, a proton pump inhibitor used to reduce stomach acid production. It is prescribed for conditions like GERD, peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Ulcers: 40 mg once daily for 2-4 weeks. Zollinger-Ellison: 40 mg twice daily, adjusted as needed.
Elderly
No specific dosage adjustment usually required, but use with caution.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. For severe impairment, monitor closely.
How to Take
Take orally, preferably in the morning, at least 30-60 minutes before a meal. Swallow whole, do not crush or chew.
Mechanism of Action
Pantoprazole irreversibly binds to the H+/K+-ATPase (proton pump) in the gastric parietal cells, inhibiting the final step in acid production. This reduces both basal and stimulated acid secretion.

Onset
Within 2.5 hours.
Excretion
Mainly renal (about 80%) and some biliary/fecal (about 20%).
Half life
Approximately 1 hour (terminal half-life), but duration of acid suppression is much longer due to irreversible binding.
Absorption
Well absorbed orally, bioavailability is about 77%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to pantoprazole or any component of the formulation
- •Concurrent use with atazanavir or nelfinavir (HIV protease inhibitors)
Drug Interactions
Warfarin
Increased INR/prothrombin time, monitor closely.
Clopidogrel
Reduced antiplatelet effect (clinical significance debated).
Methotrexate
Increased methotrexate levels, especially with high doses.
Atazanavir, nelfinavir
Reduced plasma levels, contraindications.
Iron salts, ketoconazole, itraconazole
Reduced absorption due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include drowsiness, tachycardia, nausea, headache. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk, caution advised during lactation.
