Overview of the medicine
Pacet is a brand of Paracetamol, a widely used analgesic and antipyretic drug for the relief of pain and fever. It is effective for mild to moderate pain.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg every 4-6 hours as needed, not exceeding 4000 mg (4g) in 24 hours.
Elderly
Similar to adult dosage; caution advised in those with hepatic or renal impairment.
Renal_impairment
Dose interval should be extended in severe renal impairment (CrCl < 10 ml/min). Consult a physician.
How to Take
Oral tablets can be taken with or without food. For faster absorption, it is often taken on an empty stomach. Follow dosage instructions on the label or from a physician.
Mechanism of Action
Paracetamol inhibits prostaglandin synthesis primarily in the central nervous system (CNS), leading to analgesic and antipyretic effects. It has weak anti-inflammatory properties peripherally.

Onset
Onset of action for analgesia is usually within 30-60 minutes.
Excretion
Excreted mainly by the kidneys as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Plasma elimination half-life is typically 1 to 4 hours, prolonged in liver dysfunction.
Absorption
Rapid and complete absorption from the gastrointestinal tract. Peak plasma concentrations are achieved within 30 to 60 minutes.
Metabolism
Primarily metabolized in the liver via conjugation with glucuronic acid and sulfate. A small amount is metabolized by cytochrome P450 to a potentially toxic intermediate, N-acetyl-p-benzoquinone imine (NAPQI), which is detoxified by glutathione.

Drug Interactions
Alcohol
Concurrent chronic alcohol use increases the risk of hepatotoxicity.
Warfarin
May enhance the anticoagulant effect of warfarin, increasing bleeding risk.
Carbamazepine, Phenobarbital, Phenytoin
May increase the risk of hepatotoxicity due to enzyme induction.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose can cause severe, potentially fatal hepatic necrosis. Symptoms include nausea, vomiting, abdominal pain, and sweating. Management involves gastric lavage, activated charcoal, and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Paracetamol is generally considered safe for use during pregnancy and lactation at recommended therapeutic doses. However, always consult a doctor before use.
