Description
Overview of the medicine
Panium 10 mg Syrup contains Pantoprazole, a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat acid-related conditions like gastroesophageal reflux disease (GERD) and peptic ulcers.
Uses & Indications
Dosage
Adults
GERD/Erosive Esophagitis: Typically 20-40 mg once daily for 4-8 weeks. For 10 mg syrup, dosage should be determined by a physician, often 10 mg once daily.
Elderly
No specific dose adjustment is usually required, but caution is advised. Consult physician.
Renal_impairment
No dose adjustment needed for mild to severe renal impairment. Consult physician.
How to Take
Take orally, preferably 30-60 minutes before a meal. Shake the bottle well before use. Use the provided measuring spoon or cup for accurate dosage.
Mechanism of Action
Pantoprazole irreversibly binds to the H+/K+-ATPase enzyme (proton pump) in the gastric parietal cells, inhibiting the final step in acid production. This reduces both basal and stimulated gastric acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression within 2-3 hours, full effect achieved within 2-3 days of continuous therapy.
Excretion
Mainly excreted renally (approximately 80%) and via biliary/fecal route (approximately 20%).
Half life
Approximately 1 hour (terminal half-life), but duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 2-2.5 hours.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with rilpivirine-containing products.
Drug Interactions
Warfarin
Increased INR and prothrombin time; monitor closely.
Clopidogrel
Potential reduction in clopidogrel's antiplatelet effect; clinical significance debated.
Methotrexate
May increase methotrexate levels, especially with high doses; consider temporary PPI discontinuation.
Atazanavir/Nelfinavir
Reduced absorption, concurrent use generally not recommended.
Drugs dependent on gastric pH for absorption
e.g., ketoconazole, iron salts, may have reduced bioavailability.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for pantoprazole overdose. Treatment should be symptomatic and supportive. Gastric lavage may be considered if recent ingestion. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed and potential benefits outweigh risks. Caution is advised during lactation as pantoprazole is excreted in human milk.
