Overview of the medicine
Panium 5 mg Injection contains a novel proton pump inhibitor that works by reducing the amount of acid produced in the stomach. It is used for short-term treatment of certain stomach and esophagus problems (such as acid reflux and ulcers) where oral therapy is not feasible.
Uses & Indications
Dosage
Adults
For most indications: 5 mg intravenously once daily, infused over 2-15 minutes. For severe conditions or hypersecretory states, dosage may be adjusted by a physician.
Elderly
No dosage adjustment is generally necessary in elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
Hepatic_impairment
In patients with severe hepatic impairment, the dose should not exceed 5 mg daily.
How to Take
Panium 5 mg Injection should be administered intravenously by a healthcare professional, either as a slow intravenous injection (over 2 minutes) or as an intravenous infusion (over 2-15 minutes) after reconstitution. Do not administer intramuscularly or subcutaneously.
Mechanism of Action
Panium selectively blocks the hydrogen-potassium adenosine triphosphatase (H+/K+ ATPase) enzyme system, also known as the proton pump, in the gastric parietal cells. This inhibition prevents the final step of acid production, leading to a profound and prolonged reduction in gastric acid secretion.
Pharmacokinetics
Onset
Acid suppression typically begins within 15-30 minutes of administration.
Excretion
Mainly excreted renally (approximately 80%), with the remainder eliminated in feces via biliary excretion.
Half life
Approximately 1-1.5 hours in healthy adults, but duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapid and complete absorption after intravenous administration, bypassing first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily via cytochrome P450 (CYP2C19 and CYP3A4) enzymes.
Side Effects
Contraindications
- •Hypersensitivity to Panium, any substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with atazanavir or nelfinavir due to significant reduction in their plasma concentrations.
Drug Interactions
Warfarin
Increased INR and prothrombin time; monitor closely.
Clopidogrel
Reduced antiplatelet activity of clopidogrel; consider alternative antiplatelet therapy.
Methotrexate
Possible increase in methotrexate levels, especially with high doses.
Drugs dependent on gastric pH for absorption (e.g., ketoconazole, iron salts)
Reduced absorption.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Reconstituted solution should be used within 24 hours.
Overdose
Symptoms of overdose are generally mild and may include drowsiness, tachycardia, nausea, headache, and blurred vision. Treatment is symptomatic and supportive. Panium is not removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Caution is advised when Panium is administered to a nursing woman as it may be excreted in breast milk.
