Overview of the medicine
Panium 50 mg tablet is a proton pump inhibitor (PPI) used to reduce the amount of acid produced in the stomach. It is prescribed for the treatment of various acid-related conditions like gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Typical dosage for GERD: 50 mg once daily before breakfast for 4-8 weeks. For peptic ulcer: 50 mg once daily for 4-8 weeks. For Zollinger-Ellison syndrome: starting dose may be higher, individualized.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is typically needed for patients with mild to moderate renal impairment. For severe impairment, caution is advised.
How to Take
Take Panium 50 mg tablet orally, once daily, preferably in the morning before breakfast. Swallow the tablet whole with water; do not chew or crush it.
Mechanism of Action
Panium selectively inhibits the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cell. This inhibition blocks the final step of acid production, leading to a profound and long-lasting decrease in gastric acid secretion.

Onset
Onset of action within 1 hour; full effect seen within 2-3 days.
Excretion
Mainly excreted via the kidneys (70-80%) and in bile/feces (20-30%).
Half life
Plasma half-life is approximately 1-2 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is about 50-60%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Known hypersensitivity to Panium or any excipients in the formulation.
- •Co-administration with nelfinavir (an antiretroviral drug).
Drug Interactions
Warfarin
Increased INR and prothrombin time.
Clopidogrel
Reduced antiplatelet effect of clopidogrel (clinical significance debated).
Methotrexate
Possible increased systemic exposure to methotrexate.
Ketoconazole/Itra(conazole)
Reduced absorption due to increased gastric pH.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and may include drowsiness, tachycardia, nausea, headache. Management is symptomatic and supportive; there is no specific antidote. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Excreted in breast milk, so caution is advised during lactation.
