Description
Overview of the medicine
Pantoprazole is a proton pump inhibitor (PPI) used to treat conditions where there is too much acid in the stomach. Pansec-40 mg Injection is an intravenous formulation primarily for patients who cannot take oral therapy.
Uses & Indications
Dosage
Adults
Typically 40 mg once daily, intravenously. In severe cases of Zollinger-Ellison Syndrome, up to 80 mg twice daily may be administered.
Elderly
No specific dosage adjustment is usually required, but use with caution due to potential decreased liver function.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment, monitor for adverse effects.
How to Take
Administer intravenously over at least 2 minutes, or as an infusion over 15 minutes. It must be reconstituted with the provided solvent (e.g., 10 ml of 0.9% NaCl) immediately before use. Do not mix with other drugs.
Mechanism of Action
Pantoprazole is a substituted benzimidazole that inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby suppressing gastric acid secretion. This leads to an increase in gastric pH and reduction in acid-related symptoms.
Pharmacokinetics
Onset
Within 15-30 minutes after IV administration.
Excretion
Mainly renal (approx. 80%) and biliary/fecal (approx. 18%).
Half life
Approximately 1 hour (terminal half-life), but its duration of action for acid suppression is much longer (up to 24 hours).
Absorption
Rapidly absorbed after oral administration; for intravenous administration, it bypasses absorption challenges and has 100% bioavailability.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with rilpivirine-containing products.
Drug Interactions
Clopidogrel
May reduce the antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
May increase methotrexate levels, especially with high doses, potentially leading to toxicity.
Iron salts, Ketoconazole, Itraconazole
Reduced absorption of these drugs due to increased gastric pH.
Atazanavir, Nelfinavir (and other antiretrovirals)
Reduced absorption of these drugs due to pantoprazole's effect on gastric pH, potentially leading to loss of virologic response.

Store below 25°C, protected from light and moisture. Do not freeze. Keep out of reach of children. Reconstituted solution stability is very limited.
Overdose
Symptoms may include drowsiness, tachycardia, nausea, headache. Treatment is symptomatic and supportive. Pantoprazole is not effectively removed by hemodialysis.

Pregnancy Category B. Use only if clearly needed. Excreted in breast milk, use with caution during lactation; consider potential risks versus benefits.
