Description
Overview of the medicine
Pansec 40 mg Tablet contains Pantoprazole, which is a proton pump inhibitor (PPI) used to treat conditions where there is too much acid in the stomach. It works by reducing the amount of acid produced by the stomach.
Uses & Indications
Dosage
Adults
Typical dose is 40 mg once daily for 4-8 weeks, depending on the condition. For H. pylori eradication, 40 mg twice daily in combination therapy.
Elderly
No specific dose adjustment is generally required, but caution is advised.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take orally, typically once daily, at least 30-60 minutes before breakfast. Do not crush, chew, or split the tablet.
Mechanism of Action
Pantoprazole is a substituted benzimidazole that inhibits the H+/K+-ATPase (proton pump) in the parietal cells of the stomach. This irreversible inhibition reduces both basal and stimulated gastric acid secretion.
Pharmacokinetics
Onset
Acid suppression begins within 1 hour; full effect within 2.5 hours.
Excretion
Mainly excreted renally (approx. 71%) and a smaller portion fecally (approx. 18%).
Half life
Approximately 1 hour (terminal half-life), but the duration of action is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration; peak plasma concentrations reached in 2-2.5 hours.
Metabolism
Extensively metabolized in the liver via cytochrome P450 system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to Pantoprazole, substituted benzimidazoles, or any component of the formulation.
Drug Interactions
Tacrolimus
May increase tacrolimus serum levels.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to CYP2C19 inhibition.
Methotrexate
May increase serum methotrexate levels, especially with high doses.
Atazanavir/Nelfinavir
Reduced absorption due to increased gastric pH.
Iron Salts, Ketoconazole, Itraconazole
Absorption may be decreased due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and include lethargy, tremor, abdominal pain, somnolence, and tachycardia. Treatment is supportive and symptomatic.
