Description
Overview of the medicine
Pantoprazole is a proton pump inhibitor (PPI) used to treat conditions where there is too much acid in the stomach. It works by reducing the amount of acid produced in the stomach.
Uses & Indications
Dosage
Adults
GERD: 40 mg once daily for up to 8 weeks. Maintenance: 20-40 mg daily. Erosive Esophagitis: 40 mg once daily for up to 8 weeks. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted based on patient response.
Elderly
Generally no dosage adjustment required, but use with caution due to potential for reduced hepatic function.
Renal_impairment
No dosage adjustment required.
How to Take
Take orally, preferably in the morning, at least 30 minutes before a meal. Do not crush, chew, or split the tablet.
Mechanism of Action
Pantoprazole irreversibly binds to the H+/K+-ATPase enzyme (proton pump) in the gastric parietal cells, inhibiting the final step in gastric acid secretion.
Pharmacokinetics
Onset
Within 2.5 hours
Excretion
Mainly through renal excretion (approx. 80%) and biliary/fecal excretion (approx. 20%).
Half life
Approximately 1 hour (terminal half-life), but the duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 2-2.5 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Known hypersensitivity to pantoprazole or any component of the formulation
- Known hypersensitivity to substituted benzimidazoles (e.g., other PPIs)
Drug Interactions
Warfarin
Increased INR and prothrombin time; monitor closely.
Clopidogrel
Potential for reduced antiplatelet effect, though clinical significance debated; use with caution.
Methotrexate
Increased and prolonged serum levels of methotrexate, especially with high doses; consider temporary withdrawal of PPI.
Atazanavir, Nelfinavir, Rilpivirine
