Description
Overview of the medicine
Pantodac 40 mg Injection is a proton pump inhibitor used to treat conditions where the stomach produces too much acid, such as severe gastroesophageal reflux disease (GERD), erosive esophagitis, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
The usual recommended dose is 40 mg once daily by intravenous infusion, administered over 2 to 15 minutes. In severe cases or Zollinger-Ellison syndrome, doses up to 80 mg twice daily may be used.
Elderly
No dosage adjustment is typically required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment or those on hemodialysis.
How to Take
Pantodac 40 mg Injection is administered intravenously by a healthcare professional, either as a slow infusion over 2-15 minutes or as an intravenous push over at least 2 minutes.
Mechanism of Action
Pantoprazole irreversibly blocks the hydrogen/potassium adenosine triphosphatase (H+/K+ ATPase) enzyme system, also known as the proton pump, in the gastric parietal cells. This action reduces gastric acid secretion.
Pharmacokinetics
Onset
Acid secretion inhibition occurs within 1 hour after administration.
Excretion
Approximately 80% is excreted via urine and 18% via feces.
Half life
Terminal elimination half-life is approximately 1 hour.
Absorption
Rapidly absorbed after intravenous administration, with peak plasma concentrations occurring quickly.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- Not recommended for patients with mild gastrointestinal complaints like dyspepsia.
Drug Interactions
Warfarin
Increased International Normalized Ratio (INR) and prothrombin time, increasing the risk of bleeding.
Methotrexate
Pantoprazole may increase serum levels of methotrexate, especially at high doses, potentially leading to toxicity.
Mycophenolate Mofetil
Reduced systemic exposure to mycophenolic acid (MPA), potentially decreasing efficacy.
HIV protease inhibitors (e.g., Atazanavir, Nelfinavir)
Reduced absorption due to increased gastric pH, leading to decreased efficacy of these antiviral drugs.
Drugs dependent on gastric pH for absorption (e.g., Ketoconazole, Itraconazole, Erlotinib)
Pantoprazole reduces gastric acidity, which can decrease the absorption and efficacy of these drugs.
Storage
Store below 25°C in a dry place. Protect from light. Do not freeze. Reconstituted solution should be used immediately or within 12 hours if stored at room temperature, or up to 24 hours if refrigerated.
Overdose
There is no known specific antidote. In case of overdose, symptomatic and supportive treatment should be initiated. Pantoprazole is not readily removed from the circulation by hemodialysis.
Pregnancy & Lactation
Pregnancy Category C. Pantoprazole should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; caution should be exercised when administered to a nursing mother.

Lab Monitoring
- Serum magnesium levels (if treatment extends beyond 3 months)
- Liver function tests (if liver abnormalities are suspected or with long-term use)
Doctor Notes
- Ensure patient history of hypersensitivity to PPIs or any components before administration.
- Monitor serum magnesium levels, particularly in patients on prolonged therapy or concomitant with other drugs that may cause hypomagnesemia.
- Consider bone density screening for patients on long-term, high-dose therapy.
- Assess for potential drug interactions, especially with warfarin, methotrexate, and pH-dependent drugs.
Patient Guidelines
- This medicine will be administered by a healthcare professional.
- Report any unusual or severe side effects to your doctor or nurse immediately.
- Complete the full course of treatment as prescribed, even if symptoms improve.
- Do not attempt to self-administer this injection.
