Overview of the medicine
Pantodac-40-mg-tablet contains pantoprazole, a proton pump inhibitor (PPI) used to reduce stomach acid production, treating conditions like GERD, erosive esophagitis, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
40 mg once daily for 4-8 weeks, depending on indication. For Zollinger-Ellison Syndrome, dosage may be higher and adjusted based on acid output.
Elderly
No specific dose adjustment is usually required, but caution is advised in elderly patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment or those on hemodialysis.
How to Take
Take orally, swallow the tablet whole with water, preferably in the morning before a meal. Do not crush, chew, or break the tablet.
Mechanism of Action
Pantoprazole irreversibly inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby blocking the final step of acid secretion and reducing gastric acid levels.
Pharmacokinetics
Onset
Acid suppression begins within 2-3 hours, but full therapeutic effect may take several days.
Excretion
Approximately 80% is excreted renally as metabolites, and the remainder is excreted in feces via biliary secretion.
Half life
Terminal elimination half-life is about 1 hour, but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after oral administration; bioavailability is approximately 77%. Peak plasma concentrations are reached in about 2-2.5 hours.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 (CYP) enzyme system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with rilpivirine-containing products.
Drug Interactions
Warfarin
Potential for increased INR and prothrombin time, requiring monitoring.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to CYP2C19 inhibition by pantoprazole.
Methotrexate
Increased and prolonged elevation of methotrexate and/or its metabolite serum levels, potentially leading to toxicity.
Iron salts, Ketoconazole, Itraconazole
Reduced absorption of these drugs due to pantoprazole-induced increase in gastric pH.
HIV Antivirals (e.g., Atazanavir, Nelfinavir)
Reduced absorption and efficacy of these drugs due to increased gastric pH.

There is no specific antidote for pantoprazole overdose. In case of overdose, treatment should be symptomatic and supportive. Pantoprazole is not readily dialyzable.
