Ceftriaxone is a broad-spectrum, third-generation cephalosporin antibiotic used to treat a variety of bacterial infections. It is administered via injection.
Uses & Indications
Dosage
Adults
1-2 g IV/IM once daily or in two divided doses, up to 4 g/day for severe infections. For 250 mg, specific indications like uncomplicated gonorrhea or surgical prophylaxis depending on protocol.
Elderly
No specific dose adjustment generally needed unless severe renal/hepatic impairment. Max 2g/day for severe renal impairment (CrCl < 10 mL/min).
Renal_impairment
Dose adjustment usually not required for mild-moderate impairment. For severe impairment (CrCl < 10 mL/min), max 2g/day.
How to Take
Intramuscular (IM) or Intravenous (IV) infusion/injection. IM injections should be deep into a large muscle. IV infusions over 30 minutes.
Mechanism of Action
Bactericidal; inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs).
Pharmacokinetics
Onset
Rapid
Excretion
Approximately 50-60% via kidneys, 40-50% via bile/feces.
Half life
6-9 hours (longer in neonates/elderly)
Absorption
Rapid and complete after IM, immediate after IV.
Metabolism
Not extensively metabolized; primarily converted to inactive metabolites in the intestine.
Side Effects
Contraindications
- Hypersensitivity to Ceftriaxone or other cephalosporins
- Neonates with hyperbilirubinemia
- Concurrent use with IV calcium-containing solutions in neonates
Drug Interactions
Aminoglycosides
Synergistic effect, but potential for renal toxicity.
Calcium-containing solutions
Fatal ceftriaxone-calcium precipitates can occur, especially in neonates.
Anticoagulants (e.g., Warfarin)
May enhance anticoagulant effect, increasing bleeding risk.
Loop diuretics (e.g., Furosemide)
Increased risk of nephrotoxicity.
Storage
Store dry powder below 30°C. Protect from light. Reconstituted solution has specific stability (e.g., 6 hours at room temperature or 24 hours refrigerated).
Overdose
Symptomatic and supportive treatment. Hemodialysis or peritoneal dialysis not effective for removal.
Pregnancy & Lactation
Pregnancy Category B. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding with caution.
