Description
Overview of the medicine
Pregabalin is an anticonvulsant and analgesic drug used to treat neuropathic pain, epilepsy, generalized anxiety disorder, and fibromyalgia.
Uses & Indications
Dosage
Adults
Initial dose 25-75 mg/day, usually divided into 2-3 doses. May be titrated up to a maximum of 300-600 mg/day depending on indication and patient response.
Elderly
Dose adjustment may be necessary in elderly patients with reduced renal function. Start with lower doses.
Renal_impairment
Dosage must be reduced significantly based on creatinine clearance. For CrCl 50-60 mL/min: 25-75 mg BID or TID; For CrCl 30-49 mL/min: 25-75 mg QD or BID; For CrCl <30 mL/min: 25-50 mg QD.
How to Take
Take orally, with or without food. Swallow the capsule whole with water.
Mechanism of Action
Pregabalin binds with high affinity to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system, thereby modulating the release of several excitatory neurotransmitters.

Within 1 week for neuropathic pain, varies for other conditions.
Excretion
Primarily renal excretion as unchanged drug.
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations occur within 1.5 hours.

Negligible metabolism in humans; approximately 98% excreted unchanged in urine.

Contraindications
- Hypersensitivity to pregabalin or any component of the formulation.
Drug Interactions
CNS Depressants (e.g., opioids, benzodiazepines, alcohol)
May enhance CNS depressant effects leading to increased sedation, respiratory depression, and coma. Concomitant use should be with caution.
Thiazolidinedione Antidiabetic Agents (e.g., pioglitazone, rosiglitazone)
May increase the risk of weight gain and peripheral edema when co-administered with pregabalin.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose include somnolence, confusion, agitation, and restlessness. Treatment is supportive and symptomatic; hemodialysis may be beneficial.
