Description
Overview of the medicine
Pregalex 50 mg capsule contains Pregabalin, which is used to treat neuropathic pain, fibromyalgia, and as an adjunctive therapy for partial onset seizures, as well as for generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Initial dose 50 mg three times daily (150 mg/day). May be increased to 100 mg three times daily (300 mg/day) within 1 week, and then up to a maximum of 300 mg twice daily or 200 mg three times daily (600 mg/day) depending on response and tolerability. Dosage varies by indication, consult prescribing information.
Elderly
Elderly patients may require a lower dose due to age-related decrease in renal function. Dosage should be adjusted based on creatinine clearance.
Renal_impairment
Dosage must be reduced in patients with impaired renal function. Specific dose adjustments are required based on creatinine clearance levels. Patients undergoing hemodialysis may need a supplemental dose.
How to Take
Pregalex capsules can be taken orally, with or without food. It should be swallowed whole with water.
Mechanism of Action
Pregabalin binds with high affinity to the alpha2-delta (α2-δ) subunit of voltage-gated calcium channels in the central nervous system. This binding modulates calcium influx into nerve terminals, thereby reducing the release of several excitatory neurotransmitters such as glutamate, norepinephrine, and substance P. This action is believed to be responsible for its anticonvulsant, anxiolytic, and analgesic effects.

Onset
Within 1 week for neuropathic pain and GAD, but may vary by individual and condition.
Excretion
Primarily renal excretion as unchanged drug. Renal clearance is directly proportional to creatinine clearance.
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations occur within 1.5 hours. Oral bioavailability is ≥ 90% and is independent of dose.
Metabolism
Negligible hepatic metabolism in humans; approximately 98% of the dose is excreted unchanged in the urine.
Side Effects
Contraindications
- •Hypersensitivity to pregabalin or any of the excipients.
Drug Interactions
ACE Inhibitors (e.g., lisinopril)
May increase the risk of angioedema when co-administered with pregabalin.
Thiazolidinediones (e.g., pioglitazone)
May lead to additive fluid retention and weight gain when co-administered with pregabalin.
CNS Depressants (e.g., opioids, benzodiazepines, alcohol)
May enhance CNS depressant effects, increasing the risk of sedation and respiratory depression.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and, rarely, coma. Treatment should be supportive and may include gastric lavage or induction of emesis. Hemodialysis may be considered in severe cases as pregabalin is dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Pregabalin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
