Description
Overview of the medicine
Pregalex 75 mg capsule contains Pregabalin, an antiepileptic drug primarily used to treat neuropathic pain, fibromyalgia, and as an adjunct therapy for partial onset seizures in adults. It is also used to treat generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Initial dose 75 mg twice daily (150 mg/day). May be increased to 150 mg twice daily after 3-7 days based on response. Max dose 300 mg twice daily (600 mg/day).
Elderly
Dose reduction may be required due to decreased renal function. Start with lower doses.
Renal_impairment
Dose adjustment necessary based on creatinine clearance. For CrCl 30-60 mL/min, 75 mg/day (single or divided dose). For CrCl <15 mL/min, 25-50 mg/day.
How to Take
Oral administration, with or without food. Swallow capsule whole with water.
Mechanism of Action
Pregabalin binds with high affinity to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system. This binding modulates the release of several neurotransmitters, including glutamate, noradrenaline, and substance P, which are involved in pain transmission and anxiety. It does not directly bind to GABA-A or GABA-B receptors or affect GABA uptake or degradation.
Pharmacokinetics
Onset
Pain relief can begin within 1 week for neuropathic pain, and within days for anxiety.
Excretion
Primarily excreted unchanged in the urine, approximately 98% of the dose.
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations occur within 1.5 hours. Oral bioavailability is ≥90% and is independent of dose.
Metabolism
Negligible hepatic metabolism (less than 0.1% of the dose).
Side Effects
Contraindications
- •Hypersensitivity to pregabalin or any excipients
- •Not recommended for children and adolescents under 18 years for neuropathic pain or GAD.
Drug Interactions
Ethanol
Increased risk of CNS depression and impairment of motor skills.
Opioids (e.g., Oxycodone)
May potentiate central nervous system (CNS) depression, leading to increased sedation, respiratory depression, and coma.
Benzodiazepines (e.g., Lorazepam)
Increased risk of CNS depression.
Thiazolidinedione antidiabetic agents (e.g., Pioglitazone, Rosiglitazone)
Increased risk of weight gain and peripheral edema when co-administered with pregabalin.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of the reach of children.
Overdose
Symptoms include somnolence, confusion, agitation, and restlessness. Treatment is supportive and may include gastric lavage and general supportive measures. Hemodialysis is effective for removing pregabalin.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding, as it is excreted in human milk.
