Description
Overview of the medicine
Prex-3 400 mg Capsule is an oral antibiotic belonging to the cephalosporin class, used to treat a wide range of bacterial infections including respiratory tract, urinary tract, and ear infections.
Uses & Indications
Dosage
Adults
400 mg once daily or 200 mg every 12 hours.
Elderly
No dose adjustment generally required unless severe renal impairment.
Renal_impairment
Dose reduction needed for CrCl < 60 mL/min.
How to Take
Administer orally. Can be taken with or without food, but taking with food may reduce gastrointestinal upset.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis, leading to cell lysis and bacterial death. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall.
Pharmacokinetics
Onset
Peak plasma concentration reached in 2-6 hours.
Excretion
Mainly excreted by the kidneys (50-60% as unchanged drug in urine).
Half life
Approximately 3-4 hours.
Absorption
Well absorbed from the gastrointestinal tract (oral bioavailability 40-50%).
Metabolism
Not extensively metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics
- History of serious hypersensitivity reactions to penicillin
Drug Interactions
Warfarin
Increased INR and bleeding risk.
Probenecid
Increases cefixime plasma levels by decreasing renal excretion.
Oral Anticoagulants
Enhanced anticoagulant effect.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be given. Gastric lavage may be indicated. Hemodialysis or peritoneal dialysis does not remove cefixime from the circulation in significant amounts.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts into breast milk; caution should be exercised when administered to a nursing mother.
