Overview of the medicine
Progut 20 mg tablet contains Rabeprazole Sodium, which belongs to a class of drugs called Proton Pump Inhibitors (PPIs). It reduces the amount of acid produced in the stomach, helping to treat and prevent acid-related conditions.
Uses & Indications
Dosage
Adults
Typical dose is 20 mg once daily. For GERD, usually for 4-8 weeks. For duodenal ulcer, 20 mg once daily for 4 weeks. For gastric ulcer, 20 mg once daily for 6 weeks. For H. pylori eradication, 20 mg twice daily with antibiotics for 7 days.
Elderly
No dosage adjustment is generally required.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Take the tablet whole with a glass of water. Do not chew or crush. It is usually taken before a meal, preferably in the morning.
Mechanism of Action
Rabeprazole acts as a proton pump inhibitor, irreversibly blocking the H+/K+-ATPase enzyme system at the secretory surface of the gastric parietal cells. This action inhibits the final step in gastric acid secretion, leading to a sustained decrease in gastric acidity.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour.
Excretion
Approximately 90% of the drug is excreted in the urine as metabolites, with the remaining 10% excreted in feces.
Half life
Plasma elimination half-life is approximately 1-2 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring in approximately 2-3.5 hours. Bioavailability is about 52% due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 enzymes CYP2C19 and CYP3A4. Its metabolites are inactive.
Side Effects
Contraindications
- Hypersensitivity to rabeprazole, substituted benzimidazoles, or any component of the formulation.
- Pregnancy and lactation (unless clearly necessary and benefit outweighs risk).
Drug Interactions
Digoxin
Increased digoxin absorption due to increased gastric pH.
Warfarin
Increased INR and prothrombin time. Monitor closely.
Clopidogrel
Potential for reduced antiplatelet effect of clopidogrel.
Atazanavir, Nelfinavir
Reduced plasma levels of these antiretrovirals; avoid co-administration.
Ketoconazole, Itraconazole
Reduced absorption of these drugs due to increased gastric pH.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Experience with overdose is limited. Symptoms may include lethargy, headache, nausea, vomiting, and diarrhea. Treatment should be symptomatic and supportive. Rabeprazole is not dialyzable.

Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate human studies. Use during pregnancy only if clearly needed. It is not known if rabeprazole is excreted in human milk, so caution is advised during breastfeeding.
