Uses & Indications
Dosage
Adults
Usually 20 mg or 40 mg once daily, taken at least one hour before a meal. Duration varies based on indication (e.g., 4-8 weeks for GERD).
Elderly
No specific dosage adjustment is generally required for elderly patients, but caution is advised in severe hepatic impairment.

How to Take

Mechanism of Action
Esomeprazole irreversibly inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby blocking the final step of acid production and leading to prolonged reduction of gastric acid secretion.

Excretion
Approximately 80% is excreted in the urine as metabolites, and the remaining 20% in the feces.

Side Effects
Contraindications
- Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time have been reported, requiring monitoring.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19.
Methotrexate
May increase and prolong serum levels of methotrexate and/or its metabolite.
Atazanavir/Nelfinavir
Reduced plasma concentrations of these antiretrovirals due to decreased gastric acidity.
