Description
Overview of the medicine
Pyrac (Omeprazole) is a proton pump inhibitor used to reduce stomach acid production. It is prescribed for conditions like heartburn, acid reflux (GERD), stomach ulcers, and to treat or prevent ulcers associated with NSAIDs.
Uses & Indications
Dosage
Adults
Typically 20 mg once daily before breakfast. For severe conditions, 40 mg once daily. H. pylori eradication: 20 mg twice daily with antibiotics for 7-14 days.
Elderly
No dose adjustment generally required, but caution in patients with hepatic impairment.
Renal_impairment
No dose adjustment needed for renal impairment.
How to Take
Take orally, preferably in the morning before a meal. Swallow capsules whole; do not chew or crush. For patients unable to swallow, capsules can be opened and contents mixed with a small amount of acidic liquid (e.g., apple juice) and consumed immediately.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby blocking the final step of acid production. This leads to a dose-dependent inhibition of both basal and stimulated acid secretion.

Excretion
About 80% of metabolites are excreted in the urine and the remainder in the feces.
Half life
0.5-1 hour (plasma), but clinical effect lasts longer due to irreversible binding.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 0.5-3.5 hours. Bioavailability is about 30-40% due to first-pass metabolism, increasing with repeated doses.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, particularly CYP2C19 and CYP3A4. Forms inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to omeprazole or any component of the formulation.
- •Concomitant use with Nelfinavir.
Drug Interactions
Warfarin
Increased INR and bleeding risk.
Tacrolimus
Increased serum tacrolimus levels.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Potential for increased methotrexate levels and toxicity.
Diazepam, Phenytoin
Increased plasma levels of these drugs.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, headache, nausea, vomiting, tachycardia, and sweating. Management is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. It is excreted in human milk; caution should be exercised when administered to a nursing mother.
