Overview of the medicine
Pyrica 25 mg Capsule contains Pregabalin, which is used to treat neuropathic pain, fibromyalgia, and generalized anxiety disorder (GAD). It is also used as an add-on therapy for partial-onset seizures in adults.
Uses & Indications
Dosage
Adults
Initial dose is usually 25-75 mg/day, divided into 2 or 3 doses. May be titrated up to 150-600 mg/day depending on indication and response. A 25 mg capsule is typically used for starting therapy or lower maintenance doses.
Elderly
Starting dose should be lower, typically 25 mg twice daily, due to potential for decreased renal function. Dose titration should be gradual.
Renal_impairment
Dose adjustment is necessary based on creatinine clearance. For creatinine clearance <60 mL/min, the dose should be reduced. For example, with CrCl 30-60 mL/min, maximum dose 300 mg/day.
How to Take
Take orally with or without food. Swallow the capsule whole with water. Do not crush, chew, or open the capsule.
Mechanism of Action
Pregabalin binds with high affinity to the alpha2-delta (α2-δ) subunit of voltage-gated calcium channels in the central nervous system. This binding modulates the release of several excitatory neurotransmitters, thereby exerting its analgesic, anxiolytic, and anticonvulsant effects.

Onset
Within 1-2 hours for some effects, but full therapeutic effect may take several weeks.
Excretion
Primarily excreted renally as unchanged drug.
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed following oral administration, with peak plasma concentrations occurring within 1.5 hours. Oral bioavailability is ≥90% and is not affected by food.
Metabolism
Negligibly metabolized in humans; approximately 98% of the dose is recovered unchanged in the urine.
Side Effects
Contraindications
- •Known hypersensitivity to pregabalin or any component of the formulation.
- •Severe hereditary problems of galactose intolerance, total lactase deficiency or glucose-galactose malabsorption.
Drug Interactions
Thiazolidinediones (e.g., Pioglitazone, Rosiglitazone)
Increased risk of weight gain and peripheral edema when co-administered with pregabalin. Monitor for fluid retention and heart failure symptoms.
CNS Depressants (e.g., Alcohol, Benzodiazepines, Opioids)
Increased risk of CNS depression (somnolence, respiratory depression). Concomitant use should be with caution.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and restlessness. Treatment is supportive and symptomatic; hemodialysis may be considered in severe cases as pregabalin is dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pregabalin is excreted into human milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
