Description
Overview of the medicine
Pregabalin is an anticonvulsant and neuropathic pain medication used to treat neuropathic pain, fibromyalgia, partial onset seizures, and generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Start with 75 mg twice daily or 50 mg three times daily, adjusted gradually based on response and tolerability, up to a maximum of 600 mg/day in divided doses.
Elderly
Lower starting doses and slower titration may be considered due to potential for reduced renal function.
Renal_impairment
Dosage must be reduced based on creatinine clearance (CrCl). For CrCl 30-60 mL/min, maximum 300 mg/day; for CrCl 15-30 mL/min, maximum 150 mg/day; for CrCl <15 mL/min, maximum 75 mg/day.
How to Take
Oral administration, with or without food. Swallow the capsule whole with water.
Mechanism of Action
Pregabalin binds to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system, reducing the release of several neurotransmitters like glutamate, noradrenaline, and substance P. This results in reduced neuronal excitability.
Pharmacokinetics
Onset
Pain relief may begin within 1 week for some conditions, while anxiety reduction may be faster. Full effects may take several weeks.
Excretion
Excreted primarily by the kidneys as unchanged drug. Renal clearance is directly proportional to creatinine clearance.
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 1 hour. Bioavailability is ≥ 90% and is not affected by food.
Metabolism
Negligible hepatic metabolism; less than 0.1% of the dose is metabolized in humans.
Side Effects
Contraindications
- •Hypersensitivity to pregabalin or any component of the formulation.
Drug Interactions
Thiazolidinediones (e.g., pioglitazone, rosiglitazone)
May increase the risk of weight gain and peripheral edema when used concurrently.
CNS Depressants (e.g., alcohol, benzodiazepines, opioids)
Increased risk of CNS depression, including respiratory depression, coma, and death. Use with caution.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and rarely, coma. Treatment is supportive and symptomatic. Hemodialysis can effectively remove pregabalin from plasma.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pregabalin is excreted into breast milk; breastfeeding is not recommended.
