Description
Overview of the medicine
Rabeprazole is a proton pump inhibitor (PPI) used to reduce stomach acid production. It treats conditions such as gastroesophageal reflux disease (GERD), peptic ulcer disease, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4-8 weeks. For duodenal ulcer: 20 mg once daily for 4 weeks. For H. pylori eradication: 20 mg twice daily with appropriate antibiotics for 7 days.
Elderly
No dose adjustment generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment required for patients with renal impairment.
How to Take
Take the tablet whole, do not crush or chew. It is generally recommended to take Rabeprazole once daily, before a meal (e.g., breakfast), to achieve optimal acid suppression.
Mechanism of Action
Rabeprazole irreversibly blocks the H+/K+-ATPase enzyme (proton pump) in the gastric parietal cells, thereby inhibiting the final step of acid secretion.
Pharmacokinetics
Onset
Acid-suppressive effect begins within 1 hour, maximal effect by 2-4 hours.
Excretion
Excreted mainly via urine (approximately 90%) and a small amount in feces.
Half life
Plasma half-life is approximately 1-2 hours, but its acid-suppressive effect is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations occur within 2-4 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to rabeprazole or any components of the formulation.
- •Hypersensitivity to other benzimidazole proton pump inhibitors.
Drug Interactions
Digoxin
Increased digoxin levels due to increased gastric pH.
Clopidogrel
Rabeprazole may reduce the antiplatelet effect of clopidogrel.
Methotrexate
Increased methotrexate levels, especially with high doses.
Atazanavir, Nelfinavir, Ritonavir
Reduced absorption of antiretrovirals, potentially leading to loss of efficacy.
Iron salts, Ketoconazole, Itraconazole
Altered absorption due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include drowsiness, tachycardia, headache. Treatment is symptomatic and supportive. Rabeprazole is not dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Excreted in breast milk, so caution is advised during breastfeeding.
