Overview of the medicine
Ranzith 250 mg capsule contains Azithromycin, a macrolide antibiotic used to treat a wide range of bacterial infections including respiratory tract, skin, and sexually transmitted infections.
Uses & Indications
Dosage
Adults
For most infections, 500 mg once daily for 3 days, or 500 mg as a single dose on day 1 followed by 250 mg once daily for 4 days. For uncomplicated Chlamydia, a single 1 g dose.
Elderly
No specific dosage adjustment is recommended for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment is necessary for mild to moderate renal impairment (creatinine clearance > 10 mL/min). Exercise caution in severe renal impairment.
How to Take
Take orally, with or without food. To minimize gastrointestinal side effects, it may be taken with food. Do not chew or crush the capsule.
Mechanism of Action
Azithromycin works by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting protein synthesis and preventing bacterial growth.

Peak plasma concentrations in 2-3 hours.
Excretion
Primarily excreted unchanged in bile; a small amount (approximately 6%) is excreted in urine.
Half life
Terminal elimination half-life is 2-4 days due to extensive tissue distribution and slow release.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%. Peak plasma concentrations are reached in 2-3 hours.
Metabolism
Primarily metabolized in the liver, with significant first-pass metabolism, into inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Azithromycin, Erythromycin, or any macrolide or ketolide antibiotic
- History of cholestatic jaundice/hepatic dysfunction associated with prior Azithromycin use
Drug Interactions
Digoxin
Increased Digoxin levels; monitor Digoxin levels.
Antacids
Reduced peak plasma concentrations of Azithromycin; take Azithromycin at least 1 hour before or 2 hours after antacids.
Warfarin
Increased anticoagulant effect, monitor INR.
Nelfinavir
Increased Azithromycin concentrations.
Ergot alkaloids (e.g., Ergotamine)
Potential for acute ergot toxicity.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.

Symptoms may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Gastric lavage and general supportive measures are indicated. Hemodialysis is not effective.
