Description
Overview of the medicine
Ranzith 500 mg Tablet is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
For most infections, 500 mg once daily for 3 days, or 500 mg on day 1 followed by 250 mg once daily for 4 days. For certain STIs, a single 1 g dose.
Elderly
No dosage adjustment needed for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment necessary for mild to moderate renal impairment (creatinine clearance > 10 mL/min). Use with caution in severe renal impairment.
How to Take
Administer orally, with or without food. For better absorption, it is often recommended to take it on an empty stomach (1 hour before or 2 hours after meals), but it can be taken with food to reduce gastrointestinal upset.
Mechanism of Action
Azithromycin works by binding to the 50S ribosomal subunit of susceptible bacteria, thereby inhibiting protein synthesis and consequently bacterial growth.

Onset
Typically within 2-3 hours.
Excretion
Mainly excreted unchanged in bile; a small amount is excreted via urine.
Half life
Terminal elimination half-life is 2-4 days (40-68 hours).
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%.
Metabolism
Partially metabolized in the liver, primarily by N-demethylation.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
Increased serum digoxin levels. Monitor digoxin levels.
Warfarin
Increased anticoagulant effect. Monitor INR.
Nelfinavir
Increased azithromycin serum concentrations. Monitor for azithromycin side effects.
Antacids containing aluminum or magnesium
May decrease peak serum levels of azithromycin. Administer azithromycin at least 2 hours before or 4 hours after antacids.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
