Renal_impairment
Dosage adjustment is required for patients with severe renal impairment (creatinine clearance < 30 ml/min).
How to Take
Take orally, with or without food. Shake the bottle well before each use. Use the provided measuring device for accurate dosing.
Mechanism of Action
Amoxicillin is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Approximately 30-60 minutes.
Excretion
Primarily excreted unchanged by the kidneys (50-70% within 6 hours).
Half life
1 to 1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract; bioavailability is 70-90%. Peak plasma concentrations are reached in 1-2 hours.
Metabolism
Limited metabolism; approximately 10-25% is metabolized to an inactive penicilloic acid.
Side Effects
Contraindications
- Hypersensitivity to penicillin or any other beta-lactam antibiotics
- Infectious mononucleosis (increased risk of rash)
Drug Interactions
Warfarin
May increase the anticoagulant effect.
Allopurinol
May increase the incidence of skin rash.
Methotrexate
May reduce methotrexate excretion, leading to increased toxicity.
Oral Contraceptives
May reduce the efficacy of oral contraceptives (rarely significant).
Storage
Store dry powder below 30°C, protected from light and moisture. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard any unused portion after the specified period.
Overdose
Symptoms of overdose include gastrointestinal effects (nausea, vomiting, diarrhea) and disturbances in fluid and electrolyte balance. In severe cases, renal impairment, crystalluria, or convulsions may occur. Management involves gastric lavage, symptomatic treatment, and supportive measures. Hemodialysis can remove amoxicillin from circulation.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe during pregnancy, but use with caution and only if clearly needed. Amoxicillin is excreted in breast milk; use with caution in nursing mothers, monitoring for potential adverse effects in the infant.
