Description
Overview of the medicine
Remamox 250 mg Capsule contains Amoxicillin, a penicillin-type antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Standard adult dose: 250-500 mg every 8 hours, or 500-875 mg every 12 hours, depending on the severity and type of infection. For severe infections, up to 1g three times daily.
Elderly
No specific dose adjustment for elderly patients with normal renal function. Dose adjustments may be necessary based on renal impairment.
Renal_impairment
Dosage reduction is required for patients with severe renal impairment (creatinine clearance < 30 mL/min). Consult a physician for specific dosing.
How to Take
Take the capsule orally with or without food. Swallow whole with a glass of water. Do not crush or chew. Complete the full course of medication even if symptoms improve.
Mechanism of Action
Amoxicillin is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It prevents the cross-linking of peptidoglycan polymers, which are crucial components of the bacterial cell wall, leading to cell lysis and death.
Pharmacokinetics
Onset
Antibacterial effect typically observed within 1-2 hours after administration.
Excretion
Primarily excreted unchanged in the urine by tubular secretion and glomerular filtration. Approximately 60% of an oral dose is excreted unchanged in the urine within 6-8 hours.
Half life
Approximately 1 to 1.5 hours in adults with normal renal function. This is prolonged in patients with impaired renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration. Bioavailability is approximately 70-90%. Peak plasma concentrations are achieved in about 1-2 hours.
Metabolism
Partially metabolized; about 10-25% of the dose is metabolized to inactive penicilloic acid.
Side Effects
Contraindications
- Known hypersensitivity to penicillin antibiotics (e.g., amoxicillin, penicillin G, ampicillin)
- History of severe hypersensitivity reactions (e.g., anaphylaxis) to any beta-lactam antibiotic (e.g., cephalosporins, carbapenems)
Drug Interactions
Allopurinol
Concurrent use may increase the incidence of skin rashes.
Methotrexate
Amoxicillin can reduce the renal excretion of methotrexate, potentially increasing its toxicity. Monitor methotrexate levels.
Oral contraceptives
May reduce the effectiveness of oral contraceptives. Advise using additional birth control methods.
Warfarin and other oral anticoagulants
Increased risk of bleeding due to interference with vitamin K metabolism. Close monitoring of INR is recommended.
Tetracyclines, Macrolides, Chloramphenicol
These bacteriostatic drugs may interfere with the bactericidal effect of amoxicillin.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastrointestinal symptoms like nausea, vomiting, and diarrhea may occur. Renal impairment may lead to amoxicillin crystalluria. Treatment is primarily symptomatic and supportive. Gastric lavage may be considered soon after ingestion. Hemodialysis can be used to remove amoxicillin from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Amoxicillin is excreted in breast milk; use with caution during lactation, as it may lead to sensitization or diarrhea in the infant.
