Description
Overview of the medicine
Remamox 500 mg Capsule is an antibiotic used to treat a wide range of bacterial infections. It belongs to the penicillin group of antibiotics and works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
250-500 mg every 8 hours, or 500-875 mg every 12 hours, depending on the severity and type of infection. Max 4g/day.
Elderly
Dosage adjustment may be necessary in elderly patients with impaired renal function. Otherwise, similar to adult dose.
Renal_impairment
Dosage reduction required. e.g., CrCl 10-30 mL/min: 250-500 mg every 12 hours. CrCl < 10 mL/min: 250-500 mg every 24 hours.
How to Take
Administer orally with or without food. Can be taken with food to reduce potential gastrointestinal upset. Swallow the capsule whole with water.
Mechanism of Action
Amoxicillin works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located in the bacterial cell wall, which prevents the cross-linking of peptidoglycan polymers, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Approximately 30 minutes to 1 hour (antibacterial effect)
Excretion
Primarily renal, via tubular secretion and glomerular filtration. Approximately 60-80% excreted unchanged in urine within 6 hours.
Half life
1 to 1.5 hours in adults with normal renal function
Absorption
Rapidly and well absorbed from the gastrointestinal tract, achieving peak plasma concentrations in 1-2 hours. Bioavailability is approximately 70-90%.
Metabolism
Limited hepatic metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to amoxicillin, other penicillins, or any component of the formulation
- History of severe hypersensitivity reaction to any other beta-lactam antibacterial agent (e.g., cephalosporins, carbapenems)
Drug Interactions
Warfarin
May prolong prothrombin time (INR) due to altered gut flora, requiring monitoring.
Probenecid
Increases amoxicillin levels and prolongs its half-life by reducing renal tubular secretion.
Methotrexate
May increase methotrexate toxicity due to reduced renal clearance.
Oral Contraceptives
May reduce efficacy of oral contraceptives (rarely significant, but advise backup method).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, diarrhea, and electrolyte imbalance. In severe cases, renal impairment or convulsions may occur. Treatment is supportive; hemodialysis can remove amoxicillin from the circulation.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe for use during pregnancy when clearly needed. Small amounts are excreted in breast milk; generally considered safe during lactation but monitor for diarrhea or candidiasis in infants.
