Description
Overview of the medicine
Renafor-B 100 mcg Inhalation Capsule contains Budesonide, a potent synthetic corticosteroid used for the prophylactic management of asthma and maintenance treatment of Chronic Obstructive Pulmonary Disease (COPD). It works by reducing inflammation in the airways.
Uses & Indications
Dosage
Adults
For asthma: Typically 100-400 mcg twice daily. May be initiated with 200 mcg twice daily. For COPD: Usually 200-400 mcg twice daily.
Elderly
No specific dose adjustment is generally required, but monitor for systemic side effects.
Renal_impairment
No specific dose adjustment for renal impairment. Use with caution in severe hepatic impairment.
How to Take
For inhalation only. The capsule should be placed in a dry powder inhaler device (e.g., Rotahaler) and the contents inhaled through the mouth. Do not swallow the capsule. Rinse mouth with water after each use to minimize the risk of oral candidiasis.
Mechanism of Action
Budesonide is an anti-inflammatory corticosteroid that exhibits potent glucocorticoid activity and weak mineralocorticoid activity. It works by binding to glucocorticoid receptors in the airway cells, reducing the synthesis and release of inflammatory mediators like prostaglandins, leukotrienes, and cytokines. This leads to a decrease in airway inflammation, swelling, and mucus production, thus improving lung function and reducing symptoms of asthma and COPD.
Pharmacokinetics
Onset
Initial therapeutic effects within 24 hours; maximal benefit observed after 1-2 weeks of regular use.
Excretion
Approximately 60% of an intravenous or orally administered dose is excreted via urine and feces as metabolites.
Half life
Plasma half-life is approximately 2 to 3 hours.
Absorption
Rapidly absorbed after inhalation; systemic bioavailability is low (approximately 10-20%) due to extensive first-pass metabolism in the liver.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 3A4 (CYP3A4) enzyme system to inactive metabolites.
Side Effects
Contraindications
- Acute bronchospasm or status asthmaticus (this medicine is not for rapid relief of acute symptoms).
- Hypersensitivity to Budesonide or any component of the formulation.
- Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
Drug Interactions
Other Corticosteroids
Potential for additive systemic corticosteroid effects if used concurrently with other systemic corticosteroids.
CYP3A4 Inhibitors (e.g., Ketoconazole, Itraconazole, Ritonavir)
Concomitant administration with potent CYP3A4 inhibitors can increase systemic exposure to budesonide, potentially leading to increased risk of systemic corticosteroid side effects. Exercise caution and consider dose adjustment.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children. Do not refrigerate or freeze.
Overdose
Acute overdose with inhaled budesonide is unlikely to cause clinically significant problems. Chronic overdose may lead to signs and symptoms of hypercorticism, including adrenal suppression. Management includes gradual dose reduction and symptomatic treatment.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown an adverse effect on the fetus, but there are no adequate and well-controlled studies in pregnant women. Budesonide is generally considered safe during pregnancy when benefits outweigh risks. Low levels are excreted in breast milk; however, the amount is unlikely to be harmful to the breastfed infant at therapeutic doses. Use with caution during lactation.

Lab Monitoring
- Monitor for signs of adrenal suppression if high doses are used for prolonged periods.
- Regular growth monitoring in children and adolescents.
- Periodic eye examinations for cataracts and glaucoma, especially in long-term users.
Doctor Notes
- Emphasize proper inhalation technique to optimize drug delivery and minimize local side effects.
- Counsel patients on the importance of regular use for maintenance and not for acute symptom relief.
- Be aware of potential drug interactions, especially with strong CYP3A4 inhibitors.
Patient Guidelines
- Use the inhaler correctly as instructed by your doctor or pharmacist.
- Rinse your mouth with water and spit it out after each dose to reduce the risk of oral thrush.
- This medicine is for maintenance therapy and not for relieving sudden breathing problems. Always carry your rescue inhaler for acute symptoms.
- Do not stop using Renafor-B suddenly without consulting your doctor, as this may worsen your condition.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Renafor-B is unlikely to affect your ability to drive or operate machinery.
Lifestyle Advice
- Avoid exposure to asthma triggers such as smoke, pollen, dust mites, and pet dander.
- Quit smoking if you are a smoker, as it significantly worsens respiratory conditions.
- Maintain a healthy lifestyle with regular exercise and a balanced diet.
