Overview of the medicine
Renafor-B 80 mcg Inhaler contains Budesonide, an inhaled corticosteroid used for the regular maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD). It works by reducing inflammation in the airways.
Uses & Indications
Dosage
Adults
Asthma: Initially 1-2 inhalations (80-160 mcg) twice daily. Max 8 inhalations (640 mcg) twice daily. COPD: 2 inhalations (160 mcg) twice daily.
Elderly
No specific dose adjustment is required; refer to adult dosage.
Renal_impairment
No dose adjustment is generally required.
How to Take
For oral inhalation only. Shake the inhaler well before use. Inhale slowly and deeply through the mouth, pressing down on the canister while inhaling. Hold breath for 5-10 seconds, then exhale slowly. Rinse mouth with water and spit out after each use to minimize the risk of oral candidiasis.
Mechanism of Action
Budesonide is a glucocorticosteroid with potent anti-inflammatory activity. It reduces inflammation in the airways, decreases bronchial hyperresponsiveness, and inhibits the release of inflammatory mediators, thereby improving lung function and reducing asthma and COPD symptoms.

Initial clinical improvement may be seen within 24 hours, but maximum benefit usually takes 1-2 weeks of regular use.
Excretion
Excreted primarily via the kidneys (approximately 60%) and feces (approximately 40%) as metabolites.

Absorption
Rapidly absorbed from the lung; systemic bioavailability is approximately 10-20% due to extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 3A4 (CYP3A4) enzyme system to inactive metabolites.

Contraindications
- Known hypersensitivity to Budesonide or any component of the inhaler.
- Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
Drug Interactions
CYP3A4 Inhibitors
Concurrent administration of strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir, itraconazole, clarithromycin) may increase systemic exposure to budesonide.

Overdose
