Description
Overview of the medicine
Respidox 200 mg tablet contains Doxofylline, a bronchodilator used to treat bronchial asthma and Chronic Obstructive Pulmonary Disease (COPD). It helps to relax the muscles of the airways, making it easier to breathe.
Uses & Indications
Dosage
Adults
Adults: 200 mg to 400 mg twice daily, depending on severity and patient response. Maximum 1200 mg/day.
Elderly
Elderly: Lower doses may be required, generally 200 mg twice daily, due to potentially reduced metabolism and clearance.
Renal_impairment
Renal impairment: No specific dosage adjustment required, but monitor carefully for adverse effects.
How to Take
Respidox 200 mg tablet should be taken orally with or without food. Swallow the tablet whole with a glass of water. Do not chew, crush, or break the tablet.
Mechanism of Action
Doxofylline is a methylxanthine derivative that acts as a phosphodiesterase inhibitor, leading to increased intracellular cyclic AMP (cAMP) levels. This results in the relaxation of bronchial smooth muscles, causing bronchodilation. It also exhibits some anti-inflammatory properties.
Pharmacokinetics
Onset
Bronchodilatory effect typically observed within 30-60 minutes.
Excretion
Primarily excreted via urine (less than 4% as unchanged drug).
Half life
Approximately 6-8 hours, which is longer than Theophylline.
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations reached within 1 hour.
Metabolism
Extensively metabolized in the liver (more than 90%) into inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Doxofylline or other xanthine derivatives (e.g., Theophylline, caffeine)
- •Acute myocardial infarction
- •Severe hypotension
- •Lactation (breastfeeding mothers)
- •Patients with severe arrhythmia
Drug Interactions
Furosemide
Doxofylline may have a diuretic effect, which can be additive to furosemide.
Phenobarbital, Phenytoin, Rifampicin
May decrease Doxofylline plasma levels due to enzyme induction, potentially reducing efficacy.
Other xanthine derivatives (e.g., Theophylline)
Concurrent use is generally not recommended due to increased risk of adverse effects.
Erythromycin, Troleandomycin, Lincomycin, Cimetidine, Propranolol, Allopurinol
May increase Doxofylline plasma levels and risk of toxicity. Dose reduction may be necessary.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep all medicines out of the reach and sight of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, epigastric pain, headache, irritability, restlessness, tremor, insomnia, and in severe cases, cardiac arrhythmias, hypotension, and generalized tonic-clonic seizures. Treatment involves symptomatic and supportive measures, including gastric lavage and activated charcoal to reduce absorption.
Pregnancy & Lactation
Pregnancy: Use Doxofylline during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Consult your doctor. Lactation: Doxofylline is contraindicated in breastfeeding mothers as it is excreted into breast milk and may cause adverse effects in the infant.
