Overview of the medicine
Respidox 400 mg Tablet is a broad-spectrum macrolide antibiotic used to treat various bacterial infections, particularly those of the respiratory tract, skin, and soft tissues. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
The usual dosage is 400 mg once daily for 3 to 7 days, depending on the type and severity of the infection. For Chlamydia trachomatis, a single 400 mg dose may be sufficient.
Elderly
No specific dose adjustment is required for elderly patients; however, caution is advised due to potential age-related decline in renal and hepatic function.
Renal_impairment
For patients with severe renal impairment (creatinine clearance < 10 mL/min), dose adjustment may be necessary. Consult a physician.
How to Take
Respidox 400 mg Tablet should be taken orally. It can be taken with or without food. To minimize gastrointestinal upset, it may be taken with food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Respidozin, the active ingredient in Respidox, inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms. This prevents the translocation of peptide chains, thereby stopping bacterial growth and reproduction.
Pharmacokinetics
Onset
Antibacterial effects typically begin within 1-2 hours of the first dose, with clinical improvement observed within 24-48 hours.
Excretion
Mainly excreted unchanged via the bile and feces (approximately 80%), with a smaller portion (around 10%) excreted renally.
Half life
The elimination half-life is approximately 8-12 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally; bioavailability is approximately 40-50%. Peak plasma concentrations are reached within 2-4 hours after administration.
Metabolism
Primarily metabolized in the liver, mainly by the cytochrome P450 3A4 (CYP3A4) enzyme system.
Side Effects
Contraindications
- •Known hypersensitivity to respidozin, any other macrolide antibiotic, or any component of the formulation.
- •Patients with a history of cholestatic jaundice/hepatic dysfunction associated with prior macrolide use.
- •Concomitant use with drugs known to prolong the QT interval (e.g., cisapride, terfenadine, astemizole, pimozide).
Drug Interactions
Statins (e.g., Simvastatin)
Increased risk of myopathy and rhabdomyolysis.
Anticoagulants (e.g., Warfarin)
Increased anticoagulant effect, monitor INR.
CYP3A4 inducers (e.g., Rifampicin)
May decrease respidozin levels.
CYP3A4 inhibitors (e.g., Ketoconazole)
May increase respidozin levels.
P-glycoprotein substrates (e.g., Digoxin)
Increased plasma levels of P-glycoprotein substrates.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include severe gastrointestinal upset (nausea, vomiting, diarrhea) and hearing loss. Management involves supportive and symptomatic treatment. Gastric lavage and activated charcoal may be considered. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Respidozin is excreted in human milk; use with caution during lactation.
