Overview of the medicine
Reumafen 100 mg suspension is an oral non-steroidal anti-inflammatory drug (NSAID) containing Diclofenac Sodium. It is used for the relief of pain, inflammation, and fever associated with various conditions like arthritis, muscle pain, and dysmenorrhea.
Uses & Indications
Dosage
Adults
50-150 mg daily in divided doses. For suspension, it's typically 50 mg (2.5 mL) to 100 mg (5 mL) twice daily or as directed by physician.
Elderly
Start with lower doses and monitor for adverse effects. Max 100 mg daily.
Renal_impairment
Use with caution, reduced dosage may be required. Avoid in severe renal impairment.
How to Take
Oral administration. Take with or after food to minimize gastrointestinal upset. Shake the suspension well before use.
Mechanism of Action
Diclofenac inhibits cyclooxygenase (COX-1 and COX-2) enzymes, which are involved in the synthesis of prostaglandins. Prostaglandins mediate inflammation, pain, and fever, so by inhibiting their production, diclofenac reduces these symptoms.

Pain relief usually within 30 minutes to 1 hour.
Excretion
Excreted through urine (approx. 60%) and bile (approx. 35%).
Half life
Approximately 1-2 hours (plasma elimination half-life).
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Primarily metabolized in the liver by hydroxylation and glucuronidation, mainly by CYP2C9 and CYP2C8.

Drug Interactions
Lithium
Increased plasma lithium levels.
Warfarin
Increased risk of bleeding.
Methotrexate
Increased methotrexate toxicity.
Corticosteroids
Increased risk of GI ulceration.
Diuretics/ACE inhibitors
Reduced antihypertensive effect.

Symptoms may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management involves supportive and symptomatic treatment. Gastric lavage and activated charcoal may be considered if ingested recently.

Pregnancy Category C (D in 3rd trimester). Avoid in the third trimester due to potential fetal cardiovascular effects. Use with caution during lactation; diclofenac is excreted in breast milk in small amounts. Consult a doctor.
