Description
Overview of the medicine
Reumafen 200 mg Tablet contains Celecoxib, a selective COX-2 inhibitor, used to relieve pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis.
Uses & Indications
Dosage
Adults
Osteoarthritis: 200 mg once daily or 100 mg twice daily. Rheumatoid Arthritis: 100-200 mg twice daily. Ankylosing Spondylitis: 200 mg once daily or 100 mg twice daily, may increase to 400 mg daily. Acute Pain/Dysmenorrhea: 400 mg initial dose, followed by 200 mg if needed on the first day, then 200 mg twice daily.
Elderly
Generally no dosage adjustment needed, but caution advised. Start with the lowest effective dose.
Renal_impairment
Mild to moderate impairment: No dosage adjustment. Severe impairment: Not recommended.
How to Take
Oral. Can be taken with or without food. Taking with food may help reduce gastrointestinal upset, though celecoxib generally has less GI irritation than non-selective NSAIDs.
Mechanism of Action
Celecoxib selectively inhibits cyclooxygenase-2 (COX-2), an enzyme responsible for the synthesis of prostaglandins, which mediate pain, inflammation, and fever. By inhibiting COX-2, celecoxib reduces these symptoms while minimizing gastrointestinal side effects associated with non-selective NSAIDs.
Pharmacokinetics
Onset
Pain relief usually starts within 30-60 minutes.
Excretion
Excreted mainly via feces (57%) and urine (27%) as metabolites.
Half life
Approximately 8-12 hours.
Absorption
Well absorbed orally, peak plasma concentrations typically within 2-3 hours after a single dose.
Metabolism
Primarily metabolized in the liver by cytochrome P450 2C9 (CYP2C9) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to celecoxib or sulfonamides
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Coronary artery bypass graft (CABG) surgery
- Severe heart failure
- Active gastrointestinal bleeding or ulceration
- Severe hepatic impairment
Drug Interactions
Lithium
Increased plasma lithium levels.
Warfarin
Increased risk of bleeding.
Fluconazole
Increased celecoxib plasma concentrations.
ACE inhibitors/ARBs
May reduce antihypertensive effect and increase risk of renal impairment.
Furosemide/Thiazide diuretics
May reduce diuretic and antihypertensive effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management involves supportive and symptomatic care, gastric lavage, and activated charcoal within one hour of ingestion. Dialysis is unlikely to be beneficial.
Pregnancy & Lactation
Pregnancy Category C (first and second trimesters); Category D (third trimester). Avoid in late pregnancy due to potential for premature closure of the ductus arteriosus. Use only if benefits outweigh risks. Lactation: Not recommended, as celecoxib is excreted in breast milk.
