Description
Overview of the medicine
Revira (Rabeprazole) is a proton pump inhibitor (PPI) used to treat conditions caused by too much stomach acid, such as gastroesophageal reflux disease (GERD), erosive esophagitis, duodenal ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Typically 20 mg once daily for 4-8 weeks, depending on condition.
Elderly
No dosage adjustment generally required, but caution advised.
Renal_impairment
No dosage adjustment required.
How to Take
Take orally, preferably before a meal in the morning. Swallow whole; do not crush, chew, or split the tablet.
Mechanism of Action
Rabeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase enzyme (the proton pump) on the secretory surface of parietal cells in the stomach. This blocks the final step in gastric acid secretion, significantly reducing stomach acid levels.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Approximately 90% is excreted in urine, and the remainder in feces.
Half life
Approximately 1-2 hours (plasma elimination half-life), but duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration; bioavailability is about 52% (for 20 mg dose). Absorption is not significantly affected by food.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to rabeprazole or any components of the formulation
- Hypersensitivity to other benzimidazole substituted proton pump inhibitors
Drug Interactions
Digoxin
Increased digoxin levels due to increased gastric pH.
Warfarin
Increased INR/prothrombin time; monitor closely.
Methotrexate
Possible increased methotrexate levels.
Ketoconazole/Itraconazole
Decreased absorption due to increased gastric pH.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture.
Overdose
Experience with rabeprazole overdose is limited. Management should be symptomatic and supportive. Rabeprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Avoid during breastfeeding; animal studies show excretion in milk.
