Description
Overview of the medicine
Rislock 1 mg tablet is an atypical antipsychotic medication primarily used to treat schizophrenia, bipolar mania, and irritability associated with autistic disorder in children and adolescents.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 0.5-1 mg twice daily, gradually increase to a target range of 2-8 mg/day. Bipolar mania: Initially 2-3 mg once daily, adjusted as needed.
Elderly
Lower initial doses (e.g., 0.25-0.5 mg twice daily) and slower titration recommended due to increased sensitivity and potential for adverse effects.
Renal_impairment
Initial dose 0.25-0.5 mg twice daily, with slower titration. Max dose typically 1.5 mg twice daily.
How to Take
Rislock tablets can be taken orally with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Risperidone selectively blocks dopamine D2 and serotonin 5-HT2A receptors in the brain, thereby improving symptoms of psychosis and mood disturbances. It also has affinity for alpha1-adrenergic and H1 histaminergic receptors.
Pharmacokinetics
Onset
Clinical effects may be observed within a few days, but full therapeutic benefits may take several weeks.
Excretion
Approximately 70% excreted in urine and 14% in feces, mainly as metabolites.
Half life
Risperidone has a half-life of about 3 hours; its active metabolite, 9-hydroxyrisperidone, has a half-life of about 20-24 hours.
Absorption
Well absorbed after oral administration, peak plasma concentrations reached within 1-2 hours for risperidone and 3-4 hours for its active metabolite, 9-hydroxyrisperidone.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 to its active metabolite, 9-hydroxyrisperidone. Also undergoes N-dealkylation.
Side Effects
Contraindications
- Known hypersensitivity to risperidone or any component of the formulation
Drug Interactions
Antihypertensive agents
May increase the risk of orthostatic hypotension.
Levodopa and dopamine agonists
May antagonize the effects of levodopa and other dopamine agonists.
CNS depressants (e.g., alcohol, benzodiazepines)
May potentiate the sedative effects of risperidone.
CYP2D6 inhibitors (e.g., fluoxetine, paroxetine)
May increase plasma concentrations of risperidone and its active metabolite.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms. There is no specific antidote; treatment should be supportive and symptomatic, including gastric lavage and activated charcoal if appropriate.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Risperidone is excreted in human milk, therefore, breastfeeding should be avoided during treatment.
