Description
Overview of the medicine
Rislock 2 mg Tablet contains Risperidone, an atypical antipsychotic medication used to treat schizophrenia, bipolar disorder, and irritability associated with autistic disorder. It works by restoring the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initial 2 mg once daily, target dose 4-6 mg/day. Bipolar mania: Initial 2-3 mg once daily, target dose 1-6 mg/day. Doses greater than 16 mg/day have not been shown to be more effective.
Elderly
A starting dose of 0.5 mg twice daily is recommended, with gradual titration to an effective dose. Close monitoring is advised.
Renal_impairment
Initial dose of 0.5 mg twice daily, with careful titration. Doses should be titrated slowly, and the maximum dose should not exceed 4 mg/day.
How to Take
Rislock tablet can be taken orally, with or without food. It should be swallowed whole with water.
Mechanism of Action
Risperidone is a selective monoaminergic antagonist with high affinity for serotonin 5-HT2 and dopamine D2 receptors. It also binds to alpha1-adrenergic and, with lower affinity, to H1-histaminergic and alpha2-adrenergic receptors. It acts as an antagonist at these receptors, helping to normalize neurotransmitter activity in the brain.

Initial therapeutic effects can be observed within a few days to a week, but full therapeutic response may take several weeks.
Excretion
Approximately 70% excreted in urine (35-45% as 9-hydroxyrisperidone, 10-20% as risperidone), and 14% in feces, within one week of administration.
Half life
Risperidone: approximately 3 hours. 9-hydroxyrisperidone: approximately 20-24 hours.
Absorption
Well absorbed after oral administration, peak plasma concentration achieved within 1-2 hours for risperidone and 3-6 hours for 9-hydroxyrisperidone (active metabolite). Bioavailability is approximately 70%.
Metabolism
Extensive hepatic metabolism primarily via CYP2D6 to 9-hydroxyrisperidone (paliperidone), which is equipotent to risperidone. A minor pathway involves N-dealkylation.

Management is supportive, involving maintaining an adequate airway, oxygenation, and ventilation. Cardiovascular monitoring should be initiated immediately and should include continuous electrocardiographic monitoring to detect possible arrhythmias.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk. Neonates exposed to antipsychotic drugs during the third trimester are at risk for extrapyramidal and/or withdrawal symptoms. Risperidone is excreted in breast milk; a decision should be made whether to discontinue nursing or the drug, taking into account the importance of the drug to the mother.
