Overview of the medicine
Rislock 4 mg Tablet contains Risperidone, an atypical antipsychotic medication used to treat schizophrenia, bipolar disorder (manic or mixed episodes), and irritability associated with autistic disorder in children and adolescents.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 2 mg once daily, target dose 4-6 mg/day, maximum 16 mg/day. Bipolar mania: Initially 2-3 mg once daily, target dose 1-6 mg/day. Autism: Individualized, often starting at 0.5 mg/day.
Elderly
Lower starting doses (e.g., 0.5 mg twice daily) and slower titration are recommended due to increased sensitivity and potential for adverse effects.
Renal_impairment
Initial dose 0.5 mg twice daily, increase slowly. Use with caution. Maximum 8 mg/day.
How to Take
Rislock tablet should be taken orally, with or without food. It should be swallowed whole with a glass of water.
Mechanism of Action
Risperidone is thought to work by blocking dopamine D2 receptors and serotonin 5-HT2A receptors in the brain, which helps to improve mood, thinking, and behavior.

Onset
Therapeutic effects may begin within a few days, but full benefits may take several weeks.
Excretion
Approximately 70% excreted in urine and 14% in feces, mainly as the active fraction.
Half life
Risperidone: approximately 3 hours. 9-hydroxyrisperidone: approximately 21 hours.
Absorption
Well absorbed after oral administration. Peak plasma concentrations of risperidone occur within 1 hour; for 9-hydroxyrisperidone (active metabolite), within 3-4 hours.
Metabolism
Extensively metabolized in the liver by CYP2D6 to 9-hydroxyrisperidone, which is an active metabolite with similar pharmacological activity to risperidone.
Side Effects
Contraindications
- Known hypersensitivity to risperidone or to any of the excipients.
- Patients with severe CNS depression.

Antihypertensive agents
May enhance hypotensive effects.
CNS Depressants (e.g., alcohol, benzodiazepines)
May have additive sedative effects.
CYP2D6 Inhibitors (e.g., fluoxetine, paroxetine)
May increase risperidone plasma levels.
CYP3A4 Inducers (e.g., carbamazepine, rifampicin)
May decrease risperidone plasma levels and its active metabolite.

Overdose
Symptoms of overdose include drowsiness, sedation, tachycardia, hypotension, and extrapyramidal symptoms. Management is primarily supportive, including airway maintenance, oxygenation, and hydration. Activated charcoal may be considered.

Pregnancy Category C. Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Risperidone is excreted in breast milk; advise against breastfeeding.
