Overview of the medicine
Ronem 1gm Injection contains Meropenem, a broad-spectrum carbapenem antibiotic used to treat a wide range of severe bacterial infections.
Uses & Indications
Dosage
Adults
Typically 500 mg to 2 gm intravenously every 8 hours, depending on the type and severity of infection, and the susceptibility of the causative organism.
Elderly
Dose adjustment based on renal function is necessary in elderly patients with impaired kidney function.
Renal_impairment
Dose reduction and/or increased dosing interval are required for patients with creatinine clearance less than 50 mL/min.
How to Take
Administer by intravenous infusion over 15 to 30 minutes, or by intravenous bolus injection over 3 to 5 minutes for doses up to 1 gm.
Mechanism of Action
Meropenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration.
Excretion
Excreted primarily by the kidneys, approximately 70% as unchanged drug within 12 hours.
Half life
Approximately 1 hour in adults with normal renal function.
Absorption
Rapid and complete absorption after intravenous administration.
Metabolism
Undergoes minimal metabolism; primarily hydrolyzed to an open-ring metabolite which is inactive.
Side Effects
Contraindications
- •Known hypersensitivity to meropenem, any other carbapenem antibiotics, or any component of the formulation.
- •Patients with a history of anaphylactic reactions to beta-lactam antibiotics.
Drug Interactions
Probenecid
Probenecid competes with meropenem for active tubular secretion, leading to increased plasma concentrations and prolonged half-life of meropenem.
Valproic Acid
Concurrent use with valproic acid or divalproex sodium can significantly reduce valproic acid concentrations, potentially leading to breakthrough seizures.
Storage
Store below 30°C. Protect from light and moisture. Reconstituted solutions should be used immediately or stored as per manufacturer's instructions.
Overdose
In the event of an overdose, discontinue meropenem and institute general supportive treatment. Hemodialysis can remove meropenem and its metabolite.
Pregnancy & Lactation
Pregnancy: Use only if the potential benefit justifies the potential risk to the fetus. Lactation: Meropenem is excreted in human milk; caution should be exercised when administered to a nursing woman.
