Uses & Indications
Dosage
Adults
Typically 500 mg to 1 g IV every 8 hours, depending on the severity and type of infection. For meningitis, 1 g IV every 8 hours.
Elderly
Dosage adjustment may be required based on renal function.
Renal_impairment
Reduced dosage is necessary for patients with creatinine clearance < 50 mL/min.
How to Take
Administered intravenously (IV) over 15 to 30 minutes by infusion or as a 5-minute bolus injection (for doses up to 1 g). Reconstitution with sterile water for injection or compatible intravenous fluids.
Mechanism of Action
Meropenem is a carbapenem antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It has a high affinity for PBPs 2, 3, and 4 of *E. coli* and *P. aeruginosa*.

Side Effects
Contraindications

Drug Interactions
Probenecid
Increases the plasma half-life and concentration of meropenem.
Valproic acid

Oral anticoagulants (e.g., Warfarin)
Increased anticoagulant effect may occur, requiring INR monitoring.
Storage
Store below 25°C, protected from light. Do not freeze. Reconstituted solutions should be used immediately or stored according to manufacturer's instructions.
Overdose
