Overview of the medicine
Meropenem is a broad-spectrum carbapenem antibiotic used to treat a wide variety of bacterial infections, including complicated skin and soft tissue infections, intra-abdominal infections, and bacterial meningitis.
Uses & Indications
Dosage
Adults
Usually 500 mg to 1 g IV every 8 hours, depending on the infection. For meningitis, 2 g IV every 8 hours.
Elderly
Dosage adjustment may be needed based on renal function.
Renal_impairment
Significant dose adjustment required; e.g., for CrCl 26-50 mL/min, 1 g every 12 hours; for CrCl 10-25 mL/min, 500 mg every 12 hours.
How to Take
Intravenous (IV) infusion over 15-30 minutes, or IV bolus over 5 minutes (for doses up to 1 g).
Mechanism of Action
Meropenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Side Effects
Contraindications
- •Known hypersensitivity to meropenem, other carbapenems, or any component of the formulation.

Drug Interactions
Probenecid
Increases meropenem plasma concentrations and prolongs its half-life by inhibiting renal tubular secretion.
Valproic Acid/Sodium Valproate
May decrease valproic acid serum concentrations, leading to loss of seizure control. Co-administration generally not recommended.
Oral Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect. Close monitoring of INR is recommended.
Storage
Store below 30°C. Protect from light. Reconstituted solution stable for a limited time (e.g., 2 hours at room temperature, 12-24 hours under refrigeration).
Overdose
In case of overdose, discontinue meropenem and provide supportive treatment. Meropenem can be removed by hemodialysis.
Pregnancy & Lactation
Use in pregnancy only if clearly needed. Excreted in breast milk; caution advised during lactation.
